Legacy at Town Creek offers therapy programs to help our long-term residents maximize their quality of life and functional mobility. Our therapy team conducts a thorough evaluation of each resident to develop a plan of care based on their clinical presentation. With our therapy partner, Reliant Rehabilitation, each individual’s plan of care will ensure they can either adapt, compensate, or fully restore lost functionality.

Our approach includes:

Rehabilitation gym with full set of equipment
Clinical pathways to help direct care for more than 20 diagnostic groups
Services to adapt and help improve quality of life
Training for specialized nutrition adaptive needs
Modalities for pain, muscle re-education, edema and joint restriction
24-hour skilled nursing and medical support services

We provide therapy services to treat the whole person, including:

Physical, occupational, and speech therapy
Dysphagia therapy
Complex medical management and recovery
Fall prevention
Recovery from cardiac and pulmonary events, stroke, joint replacement, and orthopedic surgery
Dementia/Alzheimer’s care
